About the Role

We’re seeking compassionate and motivated LPNs/RNs to deliver high-quality, patient-centered care in a collaborative, mission-driven clinical environment. Responsibilities

• Provide safe, culturally sensitive, patient-centered care within RN/LPN scope of practice.
• Triage patient calls and assist with walk-ins, prioritizing care based on acuity.
• Monitor clinic flow and adjust provider schedules to support urgent care needs.
• Coordinate care across the continuum, including home care, outpatient, urgent care, and hospital settings.
• Support patients and families in accessing community resources and services.
• Facilitate and document advanced care planning conversations in the EHR.
• Serve as a resource to clinical and administrative staff; delegate tasks safely to support staff.
• Communicate clearly with the care team regarding patient status and needs.
• Contribute to development of triage protocols, patient education materials, and clinical procedures.
• Partner with nursing leadership to manage treatment room stocking and clinical supply workflow.
• Support compliance with JCAHO and other regulatory or funding requirements.
• Perform additional duties as assigned by Nursing Leadership.

Required Qualifications

• Associate or Bachelor’s Degree in Nursing
• Active RN or LPN license
• 0–2 years of experience (new graduate RNs encouraged to apply)

Preferred Qualifications

• Technically savvy or eager to learn new systems
• Experience with triage and crisis management
• Bilingual Spanish skills
