About the Role
We’re seeking compassionate and motivated LPNs/RNs to deliver high-quality, patient-centered care in a collaborative, mission-driven clinical environment. Responsibilities
- Provide safe, culturally sensitive, patient-centered care within RN/LPN scope of practice.
- Triage patient calls and assist with walk-ins, prioritizing care based on acuity.
- Monitor clinic flow and adjust provider schedules to support urgent care needs.
- Coordinate care across the continuum, including home care, outpatient, urgent care, and hospital settings.
- Support patients and families in accessing community resources and services.
- Facilitate and document advanced care planning conversations in the EHR.
- Serve as a resource to clinical and administrative staff; delegate tasks safely to support staff.
- Communicate clearly with the care team regarding patient status and needs.
- Contribute to development of triage protocols, patient education materials, and clinical procedures.
- Partner with nursing leadership to manage treatment room stocking and clinical supply workflow.
- Support compliance with JCAHO and other regulatory or funding requirements.
- Perform additional duties as assigned by Nursing Leadership.
Required Qualifications
- Associate or Bachelor’s Degree in Nursing
- Active RN or LPN license
- 0–2 years of experience (new graduate RNs encouraged to apply)
Preferred Qualifications
- Technically savvy or eager to learn new systems
- Experience with triage and crisis management
- Bilingual Spanish skills
